GROUP OTAGO CERTIFICATION AGREEMENT
Waiver of Liability and Representation of Student
- By taking this course I agree that Group Otago is not and will not be liable or responsible for any
actions or inaction, or for any direct or indirect result of any services provided by Group Otago. - In the event of any dispute related to this agreement or the services provided, it is limited to the
amount I paid for this training. - I understand that this certification process is not itself a state
recognized license to practice and does not substitute for one, nor is the Group Otago method
suitable for every client/patient. - I acknowledge that the learning process does not involve the
diagnosis or treatment of other ailments or issues, injuries, or disorders. And it is up to me to
decide whether the Group Otago process is right for the particular patient/client. - As part of taking this course I agree to strictly apply the steps I learn in the proper sequence and as
described in this training. - The failure to strictly adhere to the sequence or use the correct
steps/order may result in decertification of the Group Otago method credential and may cause
increased risk of injury to the participant taking the Group Otago class. - This Agreement shall be
governed and construed in accordance with the laws of the State of Washington, without giving
effect to any conflicts of law’s provisions, and any suit, if any is brought, must be brought in King
County Washington to the exclusion of all other Courts and venues.
